The purpose of this Website is to provide EDI connectivity information to health care providers who provide services for American Family Insurance Health policyholders. Please note EDI connectivity is not presently available for Casualty claims.
Please follow the instructions below to locate EDI connectivity information:
A. Is this a Medicare secondary payer claim?
Look at patient's health insurance card (or copy of it). If the third digit on the policy number is s, this is a Medicare Supplement claim. You should file the claim with Medicare. Medicare will forward the claim to us as a Crossover claim for Parts A and B. Please visit the Medicare Website.
B. If the claim is not a Medicare secondary payer claim, please follow the instructions for the appropriate state below.
- Look at patient's health insurance card (or copy of it). Look on front side of card for the policy number; the state code is listed at the end.
- Select the appropriate state below to proceed (state code in parentheses):
